Multnomah County Vital Records registers all Births and Deaths that occur in Multnomah County. Certified copies of these records are available for six (6) months from the date of the event.

Obtaining Certified Birth Certificates

Certified Birth certificates can be obtained from Multnomah County Vital Records for six (6) months from the date of birth. Birth records can be released to immediate family members—grandparents, parents, brother/sisters, legal representatives, or government agencies.

Multnomah County Health Department can provide you with an exact copy of the original birth certificate created by your hospital. Our certified birth certificates contain not only basic information, but also the time of birth, hospital name, and dates of birth of both parents.

If you want an exact copy of your child’s original birth certificate issued by Multnomah County, order a birth certificate within the first six months after the birth of your child. After six months, we can no longer issue certificates and you will need to order your child’s birth certificate from the Oregon Department of Human Services

Ordering information
Mail order - requests must include the full name on the record, date of birth, hospital and county of birth, and mother’s full maiden name; a return name and address, phone number and relationship to the name on the record. Send completed Birth Certificate Order Form along with appropriate fee payable to Vital Records. (Birth Certificate Order Form)

Ordering in person - bring picture ID along with the same information as ordering by mail (full name on the record, date of birth, hospital and county of birth, and mother’s full maiden name; a return name and address, phone number and relationship to the name on the record).

Obtaining Death Certificates

Certified Death certificates can be obtained from Multnomah County Vital Records for six (6) months from the date of death. Death records can be released to family members, legal representatives, government agencies, or to persons or agencies with a personal or property right.

After six months, we can no longer issue certificates and you will need to order the death certificate from the Oregon Department of Human Services www.dhs.state.or.us/publichealth/chs/.
